OmniCare Medical Supplies ("OmniCare", "we", "us") is an FDA-licensed Philippine supplier of hemodialysis machines, consumables, and water treatment systems, and a provider of turnkey dialysis center setup services. Our office is at Zone 6 Sta. Lucia, Magarao, Camarines Sur 4403, Philippines.
2. What this policy covers
This policy applies to personal information we collect through our website at omnicareph.com, including the inquiry form, and through the e-mail addresses and phone numbers published on it. The dialysis centers we build and supply are owned and operated by our clients; any patient or employee records those centers keep are governed by their own privacy policies, not this one.

We do not collect financial account details, government identifiers, or health information through the website, and we ask you not to send them through the inquiry form.
4. Our legal basis
We process inquiry details with your consent, which you give by filling in and sending the form, and because processing is necessary to take steps you request before entering a contract with us. We process analytics information on the basis of our legitimate interest in operating and promoting the website, in a way that does not override your rights.
5. Cookies and analytics
The website uses Google Analytics, which sets cookies to distinguish visitors and sessions. We use it only on omnicareph.com, and the measurements reach us as aggregate statistics. You can block or delete cookies in your browser settings, or install Google's Analytics opt-out add-on; the website works without cookies.
If our inquiry system cannot be reached when you press Send, the form opens your own e-mail application instead with your answers prepared as a message to us. In that case the message is sent by you through your own e-mail provider.
6. Who we share information with
We do not sell personal information, and we do not share it with anyone for their own marketing. We use a small number of service providers to run the website and handle inquiries:
- GitHub Pages serves the website.
- Google Analytics measures website use.
- Our customer relationship system, the CrewLogix Business Suite operated by our affiliate, stores inquiries on cloud infrastructure.
- Our e-mail provider carries messages you send to our published addresses.
These providers process information only to provide their services. We may also disclose information where the law requires it.
7. Where information is stored
Inquiries are stored in our customer relationship system hosted on cloud infrastructure, and in our e-mail. Some of our providers, including GitHub and Google, store information on servers outside the Philippines, including in the United States. Where that happens, the provider is bound by its own published data protection commitments.
8. How long we keep it
We keep inquiry records while we are discussing or carrying out a project with you, and afterwards for as long as Philippine law and ordinary business record keeping require. We delete or anonymize records that are no longer needed.
9. How we protect it
The website is served over HTTPS, and inquiries travel encrypted to our systems. Access to inquiry records is limited to the people who handle them. No method of transmission or storage is completely secure, but we take reasonable organizational, physical, and technical measures appropriate to the information we hold.
10. Your rights
Under the Data Privacy Act of 2012 you have the right to be informed about how your information is processed, to access it, to correct it, to object to processing, to have information erased or blocked where the law allows, to claim damages for violations, and to data portability. To exercise any of these rights, contact us using the details in section 13. You also have the right to file a complaint with the National Privacy Commission.
11. Children
Our website and services are directed at businesses, healthcare professionals, and investors, not at children. We do not knowingly collect information from anyone under 18. If you believe a minor has sent us personal information, contact us and we will delete it.
12. Changes to this policy
If we change this policy, we will post the new version on this page with a new effective date. Changes apply from the date they are posted.
